In order to maintain your recovery from Depression it is important to monitor your mood. Once you're depressed again (rather then just feeling low) you will probably need medication to break the cycle. By taking prompt action when things start to slide you can avoid medical intervention and get yourself back on track.
This scale was designed by Dr. Ivan Goldberg to measure changes in the severity of depression that result from psychotherapeutic or psychopharmacologic treatment. It is NOT designed to make a diagnosis of depression. If you score high on this scale and suspect that you are depressed please seek professional help immediately.
Measure your score on a weekly basis and use the results to take appropriate action. Changes in score of five or more points are significant.

Instructions
1. For each item, indicate the extent to which it is true, by ticking the appropriate box.
2. Add up the total number of ticks in each column and write the result in the Column Total boxes.
3. Multiply each Column Total by the number which appears in the box below it.
4. Add together all the scores in the bottom row for your Total Overall Score.
